Vibe Care are currently partnering with a charity who are looking for someone to manage a small team and provide comprehensive social, emotional, and practical support to individuals facing mental health and emotional well-being challenges that may jeopardize their ability to maintain stable housing.
The Role:
- Lead Needs Assessments: Conduct thorough assessments for individuals referred to the project, gathering additional information as required.
- Develop Individual Support Plans (ISP): Collaborate with the person being supported to create a personalized ISP tailored to their identified needs.
- Monitor and Update Documentation: Regularly review and update risk assessments and ISPs in collaboration with the individual being supported.
- Collaborate with Partner Organizations: Work closely with relevant agencies, such as CMHTs, Local Authorities, and housing providers, to ensure holistic support.
- Promote Service User Involvement: Engage those we support in the planning, operation, and evaluation of the service.
- Liaise with Other Agencies: Coordinate with various agencies on behalf of those we support, including participation in joint meetings and case reviews for long-term and crisis intervention.
- Facilitate Access to Information: Assist individuals in accessing relevant information and advice.
- Provide Resettlement Support: Offer practical support during transitions to new accommodations.
- Support Team Management: Assist the service manager with the supervision and appraisal of a small team.
The Ideal Candidate
- Understanding of trauma-informed practice, attachment theory, and relationship-based approaches.
- Experience working in the mental health sector or supporting individuals experiencing emotional distress.
- Demonstrated ability to involve service users in the design and development of the services they access, with a strong emphasis on listening to those with lived experiences.
- Strong communication skills.
- Knowledge of adult safeguarding practices.
- Self-awareness and a commitment to reflective practice and continuous personal development.
